Essential Qualifications for DM/MCh Courses: M.D. (Medicine/Pediatrics) for D.M. courses, For Medical Genetics Candidate with M.D. (Medicine/Pediatrics) or M.D./M.S. (Obst. & Gyne.). M.S. in Surgery for M.Ch. courses, respectively, or any other equivalent qualification recognized by Medical council of India for this purpose.

Essential Qualifications for Senior Residents (Hospital Services): M.D. (Medicine/ Paediatrics) for Cardiology, Endocrinology, Gastroenterology, Nephrology, Clinical Hematology, Genetics and Neurology; M.D. Paediatric for Paediatric Gastroenterology; For Medical Genetics M.D. (Medicine/Pediatrics) or M.D./M.S. (Obst. & Gyne.) M.S. for C.V.T.S., Endocrine Surgery, Surgical Gastroenterology/Liver Transplantation, Urology, Neurosurgery (for Neuro-ophthalmology M.S. Ophthalmology, for Neuro-otology M.S. Otology); M.D. in respective specialty for Anaesthesiology, Pathology, Radiodiagnosis, Radiotherapy. For Nuclear Medicine M.D. (Nuclear Medicine) / MBBS with DRM or any other equivalent qualification recognized by MCI; For Transfusion Medicine, M.D. (Transfusion Medicine/ Pathology) or MBBS with Ph.D. in Transfusion Medicine; for Critical Care Medicine M.D. in Anaesthesiology/ Pulmonary Medicine/ Internal Medicine or Paediatric; for Lab Hematology M.D. in Transfusion Medicine/Pathology or any other any other equivalent qualification recognized by Medical council of India can apply.

Essential Qualifications for PDF Courses: D.M. (Nephrology) for Renal Transplantation Medicine , M.Ch (Urology) for Renal Transplantation Surgery, M.Ch (Neurosurgery) for Pediatric Neurosurgery and Neurosurgical Oncology. M.Ch (Surgical Gastroenterology) for Pancreatico-Billary Surgery, D.M. (Gastroenterology) for Advanced Endoscopy and GI Physiology, M.Ch. (CVTS) for Paediatrics Cardiac Surgery/Minimally Invasive Cardiac Vascular and Thoracic Surgery and D.M. (Neurology) for Neuro Physiology, Neuro Critical Care or any other equivalent qualification recognized by Medical council of India. For PDF courses candidate should submit a write up of about 200 words to describe the work done by him/her in the area, the reason for applying for PDF course and what he/she wants to do after PDF. He/She should also give names of two referees.

Candidates applying for PDF are exempted from theory examination, but they would have to appear in an interview to be held on Monday dated 02.06.2008 in respective departments.

Age limit: Upper age limit for eligibility is 35 Years on the first day of the month of start of the Session i.e.on 01/07/2008. Age relaxation of a maximum of 5 years may be given to SC/ST/OBC candidates (only those having Domicile in the state of Uttar Pradesh). Reservation to widowed/divorced/legally-separated women who have not re-married and disabled/handicapped persons will be as per Government rules. There is no upper age limit for PDF Course.

Note : Incomplete and incorrect applications, in any respect, would be rejected.

HOW TO APPLY: The information brochure and application forms can be downloaded from the Institute’s website [http://www.sgpgi.ac.in] during Tuesday 15th April 2008 to Saturday 10th May 2008. The candidates will have to fill up the APPLICATION FORM ONLINE. After online submission a print out of filled in application form MUST be taken and sent to Executive Registrar SGPGIMS Lucknow 226014 along with all relevant documents in support of age, qualification, experience, reservation etc and duly signed by the candidate. A bank draft of Rs. 1,000/- towards examination fee should also be attached with the application form. The Bank Draft should be drawn in favour of Director, SGPGIMS, Lucknow Academic Account, drawn on SBI, SGPGI Branch, (Code No. 7789), payable at Lucknow. The last date of receiving of completed application forms along with necessary enclosures is Saturday 10th May 2008.

HOW TO KNOW YOUR ELIGIBILITY: Eligibility status of each application will be available on the same Website on Thursday 22nd May 2008.

HOW TO GET YOUR ADMIT CARD: Eligible candidates can print their Admit Card ONLINE (only once) between 22nd May to 31st May 2008, using their Password & Registration No. provided at the time of filling the application.

HOW TO KNOW YOUR RESULT: The list of candidates called for departmental test will be displayed the day following the PART A – objective written examination.
The final result of entrance exam will be available on the Institute website / Institute notice boards within one week of the entrance exam.

GENERAL INFORMATION:

1. The Theory Examination for selection of D.M./M.Ch/PDCC/PDF/Senior Residents (HS) will be held on Sunday 1st June, 2008 between 10.30 AM. to 12.00 Noon.
2. The number of seats/posts advertised may increase or decrease.
3. Reservation will apply as per rule. Reservation will be admissible only to candidates having Domicile in the state of Uttar Pradesh. Candidates having Domicile other than Uttar Pradesh will be treated as General category candidates for all purposes.
4. The Candidates must possess M.D./M.S. Degree or equivalent qualification while applying for the post.
5. The Candidate must submit proof of recognition of his/her degree from Medical Council of India, failing which application will be rejected.
6. Since the application forms are to be filled ONLINE the candidate must ensure that they fill the correct and complete information. On the basis of the information filled by the candidates ONLINE, their suitability for appearing in the entrance examination will be assessed. The institute will not be responsible if application of any candidate is rejected on the basis of information filled by him ONLINE in the application form.
7. While filling ONLINE application forms candidates must follow the instructions on the screen.
8. Incomplete application and application received after the last date will summarily be rejected.
9. In case of any dispute, the decision of the Director, SGPGIMS, Lucknow, will be final.
10. The candidate must write his name and registration no. on the back of the Bank Draft.
11. The completed application along with enclosures (print out of application form filled ONLINE along with Bank Draft and all necessary documents) should be sent latest by 10th May 2008 through courier or speed post in a envelope super scribed “Application for entrance exam of SR(HS)/DM/M.Ch/PDCC/PDF” and addressed to Executive Registrar, SGPGIMS, Raebareli Road, Lucknow 226014.
