Colonoscopy: Principles and Practice

Colonoscopy: Principles and Practice
Publisher:Wiley-Blackwell | Pages:672 | December 12, 2003 | ISBN:1405114495 | PDF | 13 MB

"This book undoubtedly represents the 'state-of-the-art' in colonoscopy [...] The language is as clear as only great experts can use, the iconography (all in colour) is superb, the bibliography up to date and precise. The editorial presentation is excellent and warrants the price. Very rarely has the reviewer examined a monograph devoted not to a disease but to a single technique which has impressed him so much for its clarity, completeness, attractiveness and – if it is allowed – its fascination."

Hospital told to go for settlement with petitioner

J. Venkatesan

He has sought Rs. 7 crore as compensation alleging medical negligence

Offer of compensation should be

reasonable: court

The complainant became a paraplegic

after surgery

NEW DELHI: Seeing the plight of a person who became a paraplegic after surgery and arguing his case from a wheelchair, the Supreme Court on Tuesday asked the Nizam’s Institute of Medical Sciences, Hyderabad, to go for a settlement with the complainant seeking a total compensation of Rs. 7 crore against Rs. 15.5 lakh awarded by the National Consumer Disputes Redressal Commission.

A Bench consisting of Justices B.N. Agrawal and G.S. Singhvi said, “the Director of the Institute, along with the State Health Secretary and the complainant or his father, shall explore the possibility of a settlement.”

While the complainant insisted on payment of Rs. 7 crore including interest, counsel for the hospital agreed to go for an out-of-court settlement, provided the Commission’s observation of negligence on the ground that a neurosurgeon was not present at the time of surgery was removed.

Mr. Justice Agrawal told counsel that the offer of compensation should be reasonable. The Bench directed that the matter be listed to July 22, when settlement or otherwise would be informed to the court.

Prasanth S. Dhananka went to the hospital in September 1990 for a check-up as he was suffering from on and off fever for one year.

On examination, it was noticed that he had a large mass in his left hemithorax (chest cavity). A surgery was performed and the tumour was removed. But, post-surgery he became a paraplegic — paralysis of the lower limbs of the body. He moved the Commission seeking a compensation of Rs. 5.83 crore, but it awarded Rs. 15.5 lakh. While the hospital filed an appeal in the Supreme Court for quashing the findings, the complainant sought an enhancement of the compensation.

The hospital contended that the Commission had erroneously held that it was negligent in not performing its statutory duties to interact and exchange opinion with sister-institutes in India and abroad and also failed to seek the assistance of a neurosurgeon.

The complainant’s main charge was that the tumour being a neurogenic one, the surgery should have been handled by a neurosurgeon. The surgery completely disturbed the nervous system, making him paraplegic. The compensation awarded was grossly inadequate, he said and sought Rs. 7 crore, including interest.

Endocrine therapy for breast cancer.

Description: Breast cancer is one of the leading causes of cancer mortality in women worldwide, and the risk of disease recurrence continues despite improvements in screening and treatment. For patients with hormone receptor-positive breast cancer, some form of endocrine therapy is central to the management of their disease. Tamoxifen has long been the mainstay of endocrine therapy in this group of patients. However, there is mounting evidence showing that the aromatase inhibitors are able to reduce overall oestrogen levels and appear to be better tolerated over a long term than tamoxifen. New and emerging indications for the aromatase inhibitors are providing clinicans (and patients) with alternative treatment options with differing side effect profiles. Part of the Oxford Oncology Library, this pocketbook will summarise the latest evidence-based and practical information on the use of endrocrine therapies in the management of breast cancer (early stage, metastatic disease, and prevention of disease). With contributions from leading oncologists from the MD Anderson Cancer Center, the book covers the range of endocrine therapies currently available to clinicians including ovarian ablation, anti-estrogens, aromatase inhibitors, fulvestrant, progestins, androgens, and combination endocrine and chemotherapy. The book also includes useful chapters on the identification and optimal selection of patients for endocrine therapy as well as the role of endocrine treatments in the prevention of breast cancer. This book will serve as an invaluable reference for all health care professionals involved in the management of patients with breast cancer.

IN DIRE STRAITS - PG medical students feel cheated
Colleges demand money for returning original certificates
Chennai

Article Rank

ABOUT 250 post-graduate medical students, waiting for their final results, felt blackmailed as they had neither the jobs they have specialised in, nor their original certificates to pursue alternatives.

Worse still is their quandary as bonded victims since they are asked to pay back money in case they want their original certificates back. Medical college authorities were withholding their original certificates demanding either cash or suffer a long wait to find a vacancy.

Fifty percent of the seats in postgraduate courses are allotted for serv ice candidates (those who are working in Government medical centres), the rest are divided among candidates from Tamil Nadu and rest of India.

Many in the 2005-06 batch of nonservice doctors had joined the medical colleges in Tamil Nadu.

Dr Lakshmi (name changed) was one such person, who got merit based seats in many centres in the country.

But, three months after joining the Madras Medical College, she was given a bond to sign that stipulated that she had to work for the Government ``if required'', else pay three lakh rupees back to the Government. Fearing a loss of one year, the students signed the bond and now they feel cheated.

According to the list put up on the date of counselling, the vacancies available for qualified MD (general medicine) in nine districts is 9, whereas the total number of non-service PGs in general medicine is 110. Also, a letter from the office of the Director of Public Health and Preventive Medicine dated June 14, 2007 to all institutional heads, had stated: ``They (PG students) are not liable for placement/transfer to PHC side, since Government orders are issued exclusively for DME/DMS (diploma holders) only.'' According to some doctors, there was no sufficient infrastructure for speciality doctors to work in tertiary (third)level centres. Health Secretary V Subburaj, however said letters would be issued very soon

Sternberg's Diagnostic Surgical Pathology

Sternberg's Diagnostic Surgical Pathology (2 Vol. Set)

Sternberg's Diagnostic Surgical Pathology (2 Vol. Set)
By Stacey E Mills, Darryl Carter, Joel K Greenson, Harold A Oberman, Victor E Reuter

Publisher: Lippincott Williams & Wilkins
Number Of Pages: 2912
Publication Date: 2004-03-01
ISBN-10 / ASIN: 0781740517
ISBN-13 / EAN: 9780781740517
Binding: Hardcover

Completely updated, the Fourth Edition of this standard-setting two-volume reference presents the most advanced diagnostic techniques and the latest information on all currently known disease entities. More than 90 preeminent surgical pathologists offer expert advice on the diagnostic evaluation of every type of specimen from every anatomic site. The Fourth Edition contains 3,494 full-color photographs, of which over 1,100 are new. This edition has three distinguished new editors--Joel K. Greenson, MD, Victor E. Reuter, MD, and Mark H. Stoler, MD--and many new contributors. Updates include new immunohistochemical markers for lymphoid neoplasms, current nomenclature for lymphoid tumors, and state-of-the-art molecular genetic tests. A bound-in CD-ROM contains all the images from the book, downloadable to PowerPoint presentations.

Summary: Abbreviation overload
Rating: 4

Of course Sternberg is one of the leading sources of general surgical pathology information and the book lives up to its reputation with adequate discussions of each topic with relatively good microscopic descriptions and information on special stains (unlike the new Rosai/Ackerman). However, consistently, he gives the name of an entity in one place in the books/volumes and then every where else it is only reffered to by its abbreviation. So now if you don't remember the abbreviation you have to hunt for it ealier in the chapter or in other chapters all together. And of course the abbreviation is not in the glossary nor is there a seperate index of abbreviations. I find that this adds unnecessary distractions and time to what should be smooth studying.

Summary: Usual excellent surgical pathology reference.
Rating: 5

An excellent summary of surgical pathology. Does need to be supplemented with monographs and articles, such as those by Odze, Fenoglio-Preiser, Barnhill, Rosen, Foucar, Harris, Gnepp, Neville, Vigorita, Stocker, Humphrey, Ro, Robboy, Connor, Burger, Ellison, Miettinen, AFIP tumor and non-neoplastic fascicles, Major Problems in Pathology, Atlases in Diagnostic Surgical Pathology, WHO blue books, and all the usual journals. I also recommend Fletcher as an additional source.
