Description: Chemotherapy, as part of a multi-disciplinary and multi-modality approach, has become the standard of care for the treatment of locally advanced squamous cell cancer of the head and neck. Until recently, chemotherapy has been delivered as either induction chemotherapy, also known as neoadjuvant therapy, or in combination with radiotherapy as chemoradiotherapy (CRT). Although induction chemotherapy with cisplatinum and 5-fluorouracil (PF) is effective in improving survival, patients treated with induction chemotherapy have a high rate of local-regional failure despite a reduced rate of distant failure. CRT has improved survival by reducing local-regional failure, with no improvement in control of distant disease.
To optimize therapy, sequential therapy approaches, combining induction chemotherapy, CRT, and surgery have been developed. In addition, recent advances in induction chemotherapy, namely the demonstration that a three drug induction chemotherapy regimen with docetaxel, cisplatinum, and 5-fluorouracil (TPF) is significantly more effective than PF, has increased interest and optimism regarding the potential gains of induction chemotherapy, as a sequential therapy approach. Preliminary data support the use of sequential therapy in patients with poor-prognosis head and neck, and a phase III trial with this schedule shows highly encouraging improvements in survival, lessened toxicity, and a significant advantage to TPF in this setting. The different treatment paradigms of sequential therapy and CRT are being compared in phase III studies. TPF has replaced PF as the standard for induction chemotherapy, and sequential therapy represents an acceptable standard of care for patients with curable, locally advanced head and neck cancer.

T�PURAM: Applications have been invited for the post-graduate super speciality courses for 2008 in Thiruvananthapuram, Kottayam and Kozhikode Medical Colleges.

The course for which applications have been invited are MCH Paediatric Surgery, MCH Plastic Surgery, MCH Geneto Urinary Surgery, MCH Thoracic Surgery, MCH Neuro Surgery, MCH Gastro-Enterology Surgery, DM Cardiology, DM Gastro-Enterology, DM Neurology and DM Nephrology.

The candidates seeking admission to these courses should apply in the prescribed format, which can be downloaded from the official website cee-kerala.org of the Commissioner for Entrance Examinations from May 18, along with a demand draft of Rs 1,500 drawn from any nationalised bank, in favour of Commissioner for Entrance Examinations payable at Thiruvananthapuram. Service quota candidates who apply for general quota also should submit a demand draft for Rs 3,000.

The applications should reach the office of the Commissioner for Entrance Examinations, Housing Board Buildings, Santhi Nagar, Thiruvananthapuram - 695001 on or before 5 p.m. on June 12.

As per Government of Uttar Pradesh Order No 623(1)/ 71-3-08 dated 25-03-2008 CSM Medical University UP, Lucknow shall be conducting Entrance examination for admission to DM/ MCh courses in CSM Medical University, Lucknow, GSVM Medical College, Kanpur and LLRM Medical College, Meerut for the year 2008, and Non DM/MCh Post Doctorate Certificate course (PDCC) in GSVM Medical College Kanpur and super-specialty Non DM/MCh Senior Residents in CSM Medical University.

Those candidates who have passed MD / MS in 2008 shall be eligible only if they produce a passing out certificate at the time of counseling.

If Non-degree Senior Resident candidates are not available in reserve category then these seats may be filled with general category candidates.

The candidate shall himself/herself arrange for his boarding and lodging during his/her stay at Lucknow. No TA or DA will be applicable

Incomplete forms and forms received after the due dates will not be entertained.

Application/Examination fee will not be refunded nor will be adjusted for any subsequent examination in any case.
