In this issue of Surgical Clinics of North America, a spectrum of liver surgery topics is summarized bringing the reader up to date with the era of modern hepatic resection surgery. The basics of liver anatomy and history are reviewed, along with the current molecular mechanisms of liver ischemia/reperfusion injury relevant to liver transplantation, hepatic trauma, and elective hepatic resections. Hepatic imaging techniques of ultrasound, CT, and MRI have improved and greatly facilitate our diagnostic capabilities for benign lesions and malignancies; however, the conundrum of the subcentimeter indeterminate liver lesion remains. The often confusing nomenclature of hepatic resection surgery was standardized at the 2000 Brisbane, Australia, International Heptao-Pancreato-Biliary Association meeting and is now known as the Brisbane 2000 terminology. Advances in the past decade include the development of laparoscopic liver resection techniques, which have been reported in more than 3000 cases worldwide. Further, robotic liver resection surgery has been described but remains in its infancy. Repair of laparoscopic cholecystectomy injury remains an uncommon but important task for the hepatobiliary surgeon, as is the potentially life-saving emergency hepatic resection for traumatic liver injury. Resection of hepatic colorectal cancer metastases has seen a paradigm shift from a prior emphasis on number of lesions to the present-day focus on sufficient liver remnant and invokes strategies of portal vein embolization and two-stage hepatectomies to increase resectability rates. Current approaches to management of hepatocellular carcinoma continue to evolve, including live donor liver transplantation, selective hepatic resection, ablative techniques, regional liver therapies, and newer systemic chemotherapy. Intrahepatic cholangiocarcinoma is on the rise, and major hepatic resections have been advocated for metastatic neuroendocrine cancers. Successful liver surgery requires a fundamental understanding of liver anatomy, disease pathophysiology, and modern hepatic resection techniques. The evaluation and management of both benign and malignant diseases of the breast are likely to demand a substantial fraction of a general surgeon's practice, yet general surgery residents frequently receive their breast pathology education in a fragmented fashion. Exposure to outpatient clinics and office-based management (where much of the decision-making processes in breast cancer take place) is limited, and surgical breast procedures are often assigned to junior trainees, who do not necessarily understand the subtle judgment that is required in planning incisions, skin flaps, and extent of breast resections. The surgical chief resident must finish his/her training program with the requisite number of breast procedures documented in their case log, but the majority of them may have been performed during the first half of the residency, and there may have been very little exposure to considerations regarding neoadjuvant therapy versus postoperative adjuvant therapy, postmastectomy radiation, and breast cancer risk reduction. Our goal was to develop a collection of articles that cover essential areas of surgical as well as nonsurgical breast cancer management. We have therefore compiled 18 articles that focus on several important aspects of breast disease management; most of the articles have been coauthored by surgery trainees so that resident-level perspectives are provided. These articles can be categorized in general as having an emphasis on one of the following areas: (1) contemporary surgical treatment strategies (eg, lymphatic mapping and breast reconstruction); (2) new systemic therapy strategies (adjuvant and neoadjuvant); (3) breast cancer risk reduction (surgical and medical); and (4) novel research directions that trainees should consider (eg, disparity-related research and breast tumor ablation techniques).

Medical imaging isn’t all that much different from creative photography in some ways. Most of the images we use don’t really look like the real subject at all. Livers aren’t some Hounsfield gray color. Pulmonary arteries don’t have bright white blood running through them. Yet, we get used to looking at these images and making the translation in our minds. Even though current imaging devices are far more sophisticated in terms of digital manipulation of the acquired data, the images are still shadows of reality. To really understand what these shadows mean, one has to do two things: understand the process by which the images are created and compare a lot of images to the gold standard of reality.

The constant fractionation of care has divided the medical community into two nonequal groups: those who look at the images they request and those who read the reports. Perhaps one of my most annoying professional disappointments is when someone calls me about a patient and tells me what the “report” of an imaging study states without having looked at the images, even when the person calling requested the study be performed. One night on call, I was asked by a calling physician to see a patient because “the CT said (sic) the patient had a bowel obstruction.” I asked if he had seen the images of the patient he was calling me about. He replied, “Why should I? I am not a radiologist.” It turned out he had not seen the patient either---disappointing on many levels.

To get the best out of imaging, we need to choose imaging studies wisely and interpret them correctly. In order to do that, we have to understand the power and limitations of imaging and its applications, especially how these studies complement clinical evaluation instead of replacing the history and physical exam. Drs Cogbill and Jarman, along with their colleagues, have assembled a collection of articles that will give the reader a much better foundation upon which to improve their understanding of imaging. Still, practice will be required in order to refine these skills. One will only see what one looks for but only if one actually looks.

in this issue of Urologic Clinics of North America, our outstanding contributors—all leaders in the field—review landmark studies that have shown that 5-alpha reductase inhibitors indeed are effective in the chemoprevention of prostate cancer. They also review other phase III studies that have raised questions about how aggressively we should screen the general population with prostate-specific antigen tests. Once prostate cancer is diagnosed, newer novel imaging techniques are allowing for better staging and—with some controversy—the possibility of guided and potentially focal therapeutics with cryotherapy and high-intensity focused ultrasound. Moreover, disruptive technologies, such as robotic-assisted laparoscopic surgery and the Internet have toppled traditional word-of-mouth referral patterns, making patients themselves drivers of early adoption of new technology. Moreover, for men opting for surgery, the appropriateness and extent of pelvic lymph node dissection remain unclear. The field of radiation therapy has also rapidly changed, and continued evidence for its efficacy is being evaluated both as primary and adjuvant therapy.

New data on systemic therapies are reviewed, including those given at the time of diagnosis and at the time of relapse, such as hormones and chemotherapy. A new immunotherapy treatment for prostate cancer will likely be commercially available in 2010. Indeed, perhaps the future of prostate cancer can be seen in new advances in the role of biomarkers and novel therapeutics, as molecular medicine begins to demonstrate promise in prostate cancer patients

Obesity has emerged as a major global health problem with disease prevalence reaching epidemic proportions. In the United States alone, obesity is responsible for more than 300,000 deaths per year. Additionally, the direct and indirect related costs for care of obesity-related disease exceed $100 billion per year. Obesity has a particular relevance to gastroenterologists given the wide spectrum of causally related disease implications specific to this specialty. It is increasingly apparent that obesity has significant implications for gastrointestinal diseases and increased risks of serious consequences, including cancer. Gastroenterologists are increasingly involved in the care of obese and overweight patients. This involvement includes assessing for appropriate screening for neoplasia, as well as conducting evaluations or interventions pre- or postoperatively.

In the first article, Drs Dang Nguyen and Hashem El-Serag define the scope of the obesity problem. Over the last 3 decades, overindulgence and obesity have been transformed from a relatively minor public health issue affecting affluent societies to a current global epidemic with major public health implications. This article puts into perspective the epidemiologic implications, in particular for gastrointestinal-related diseases.

In the next two articles, the focus is on gastrointestinal symptoms in obese patients. The role of gastrointestinal symptoms in overweight and obese individuals has only barely been explored, which is surprising given that the gastrointestinal tract is responsible for the mechanical and chemical breakdown of food for absorption by the body. Dr Guy Eslick puts into perspective the prevalence of particular gastrointestinal symptoms correlated with increased body weight. Dr Amy Foxx-Orenstein follows with an excellent summary of the physiologic explanations for this correlation of increased gastrointestinal symptoms as a function of body habitus.

Gastroesophageal reflux disease is an extremely prevalent disease and there is increasing evidence to support a causal relationship with obesity for both disease prevalence as well as complications. Drs Girish Anand and Phil Katz detail the most recent evidence for mechanistic explanations for causality and highlight the increased severity and complications of gastroesophageal reflux disease in this specific patient population.

While the most studied effect of obesity is on its association with colorectal neoplasia, lesser known effects include a heightened risk of complications of diverticulosis, a heightened risk inadequate bowel preparation, and a poorer, postoperative outcome after colon surgery. Dr Carol Burke provides a comprehensive update on the effects of obesity on colonic diseases and complications. The review highlights the emerging data showing that adverse effects of obesity on the colon promote carcinogenesis and impair wound healing. This has implications for appropriate screening as well as surgical management of the obese patient.

Nonalcoholic fatty liver disease is a very common clinical condition evident as a complication of obesity. The spectrum of liver histologic abnormalities ranges from simple steatosis to steatohepatitis, advanced fibrosis, and cirrhosis. Dr Anna Mae Diehl provides an outstanding overview of the comorbid diseases associated with obesity-related liver disease and then reviews the clinical features, management strategies, and prognosis for this extremely prevalent problem.

The medical management for obesity is a key area of interest. Pharmacologic therapy for obesity is clearly in a state of transition. Historically, there have been few effective agents, and many have been withdrawn because of unacceptable side effects. Given the widespread prevalence of obesity, there is intense interest in a nonsurgical option for treatment. Dr Lee Kaplan details the past, present, and future medications for the medical therapy option. His article focuses on key areas of interest to providers taking care of patients who are on these agents and highlights potential side effects and complications related to these medications.

Gastroenterologists are becoming increasingly involved in both the preoperative and postoperative care of bariatric patients. Accordingly, it is key for gastroenterologists (and those dealing with gastrointestinal-related issues) to understand the full spectrum of assessment and management. This ranges from both the preoperative evaluation for gastrointestinal-related disease to management of a wide array of endoscopic interventions and metabolic complications postoperatively. Dr David Greenwald begins the discussion with the gastrointestinal role in the preoperative assessment of bariatric patients. Drs Mitch Schreiner and Brian Fennerty focus in particular on the standard endoscopic assessment and management both pre- and postoperatively. Drs Tercio Lopes and Mel Wilcox then address the approach to the postoperative bariatric patient with pancreaticobiliary disease and provide insight and technical expertise dealing with the altered anatomy. Dr Tim Koch then provides an excellent overview on the wide array of metabolic complications for the postoperative bariatric patient. His review highlights the macro- and micronutrient potential deficiencies and he provides a comprehensive approach to both the short- and long-term management to treat or potentially avoid these complications.

Bariatric surgery is the most definitive treatment for obesity. As bariatric surgery has gained widespread acceptance, evidence continues to grow that it is the only treatment currently available that demonstrates significant cure rates for such diseases as diabetes, hypertension, and heart failure, as well as the myriad of other comorbid conditions associated with obesity. In 2008, at least 220,000 obesity surgeries were done in the United States, with gastric bypass being the “most popular method.” Drs Mark Fontana and Steve Wohlgemuth review the weight-loss surgery options. The emphasis is on the common operations performed currently, but they also discuss operations no longer performed. Knowledge of the anatomic changes is key to understanding and optimizing management strategies of the short- and long-term postoperative complications. Drs Paul Frank and Peter Crookes provide the surgeon's perspective for the short- and long-term follow up of the postbariatric surgery patient. They emphasize that, although early surgical follow up is concentrated on the perioperative period to ensure healing and care for any surgical complications, the key element of ultimate “surgical success” is the long-term programmatic follow up for these patients. These authors provide recommendations drawing from their extensive experience with the comprehensive program used at their institution.

Recognizably from this edition of Gastroenterology Clinics of North America, obesity is a major risk factor for a range of serious medical conditions, including a wide spectrum of gastrointestinal-related disorders and complications. The complex metabolic activity of adipose tissue results in the production of proinflammatory cytokines and neurohumoral and immune-mediated mechanisms, leading to a host of systemic effects, which include a broad array of gastrointestinal symptoms and diseases.

So where do we go from here? All medical practitioners, including gastroenterologists and bariatric surgeons, must remain aggressive about addressing and treating obesity in their patients. This monograph will, it is hoped, be a key step in advancing the knowledge base of those who care for obese patients. Furthermore, it is the collective intent of the authors that this will also highlight the absolute need for a comprehensive, well-integrated multidisciplinary and lifelong approach to these patients

As one reads this issue one will find a significant amount of material addressing operative techniques. One will also find an expanse of material relating to the molecular, biochemical, imaging, genetic, genomic, and proteomic aspects of what is known about endocrinology. I can think of few other areas of study where we have advanced as much in our ability to more fully understand the physiologic and structural aspects of pathologic target organs preoperatively as we now can in endocrine surgery. These advances have in some cases allowed us to significantly and reliably reduce our need to dissect and explore at the time of operation. Conversely, we may decide to reliably expand our initial operative plans based on gene analysis.

It has been quoted (multiple times in just this issue) that the most important localizing study in endocrine surgery is to localize a competent endocrine surgeon. The corollary for that could be that the surgeon who desires becoming facile and well-employed as an endocrine surgeon must localize an endocrinologist (or preferably group) with whom he or she can work well. Part of this relationship will likely depend on having an excellent working knowledge of the nonoperative aspects of endocrinology.

Despite the desires of the American Board of Surgery to claim that all general surgeons who are board certified have demonstrated expertise in endocrine surgery, the reality of the practice place is that these operations are almost always performed by persons with additional training and defined focus in one or more subsets of endocrine surgery. To be sure, there is overlap with otorhinolaryngologists, surgical oncologists, and hepatopancreatobiliary surgeons in the management of these patients but the reality that “specialty-” and “subspecialty”-trained surgeons perform the lion's share of these procedures is inescapable.

General surgeons will still maintain a need for knowledge in these areas. If for no other reason, endocrinopathies—both recognized and unrecognized—may be encountered in other patients and must be understood to safely manage their care. Also, we remain in a highly dynamic state regarding the delivery of health care. As events unfold, there may be a need to shift workforce supply and care distribution. Given the relative distribution of general surgeons compared with subspecialists, it is not inconceivable that some aspect of care previously relegated by degrees by some general surgeons may need to be reconsidered. A more important reason to be familiar with the larger breadth of material is because that is what is required for mastery of the topic. In an age where more people are looking for “just-in-time” information about small topics, mastery is becoming rarer. For those whom surgery is a passion, mastery is imperative.

Recurrent bleeding and death continue to be a major problem despite advances in therapeutic endoscopy and pharmacotherapy. The authors critically reviewed predictive models of poor outcome from acute upper gastrointestinal bleeding that may identify high-risk patients for timely management. The optimal management strategy for massive peptic ulcer bleeding remains a challenge - endoscopists, surgeons, and interventional radiologists often have divergent views. Leading investigators in the field discuss the options of repeated endoscopic therapy, early surgery, and angiographic embolization in high-risk patients. Other articles provide an authoritative overview of the management of stress-ulcer in critically ill patients and the approach to refractory peptic ulcer disease. In the last decade, nonsteroidal anti-inflammatory drugs (NSAIDs) and low-dose aspirin have gradually replaced Helicobacter pylori infection as the major cause of peptic ulcer disease in our aging population. Until recently, gastroenterologists and cardiologists used to have different views on strategies of preventing ulcer complications in patients requiring anti-platelet therapy. Now there is evidence that cyclo-oxygenase (COX)-2 selective and nonselective NSAIDs increase the risk of cardiothrombotic events. How to balance risks and benefits of NSAID use is an important issue for primary care doctors and rheumatologists. Experts in this field have provided critical and updated reviews on these frequently debated issues. The article entitled “Prevention of NSAID-induced ulcer: Looking to the future” provides a comprehensive update of new drugs and novel compounds that carry the potential of reducing gastrointestinal toxicity of NSAIDs. Although not widely recognized, NSAIDs can cause serious complications to the entire gastrointestinal tract. It is therefore relevant to include an updated review of NSAID-associated lower gastrointestinal complications amongst other articles on peptic ulcer disease. With a declining prevalence of H pylori, ulcers not associated with H pylori or NSAID use (non-NSAID non-H pylori ulcer) are increasingly recognized. The author provides an authoritative overview and practical approach to this diagnostic dilemma. It is with tremendous optimism that I welcome you to this issue of Gastroenterology Clinics of North America, which is devoted to esophageal cancer. Esophageal cancer is on the rise in terms of incidence in the Western world, and survival rates have not improved much in the last 30 years. This lack of progress in survival rates is mainly because, unfortunately, most individuals only develop symptoms after the cancer has already metastasized to other organs, by which time a cure is usually out of the question. Esophageal cancer is a devastating disease. As Lyman A. Brewer IIII said, “No patients with malignancy are more miserable than those suffering unrelieved malignant obstruction of the esophagus, because they ultimately die of slow starvation” (Am J Surg 1980:139:730–43). Esophageal cancer is a major public health problem, and the time has come for funding bodies (in particular, cancer organizations) to take greater notice and dedicate increased funding to all aspects of esophageal cancer research.

The topics selected for this issue of Gastroenterology Clinics of North America have been chosen for two reasons. The first is to fill gaps in knowledge. Thus, new information is presented on cutting-edge topics, such as genetic polymorphisms, and on topics unfamiliar to many in the field, such as the use of Chinese herbal medicines. The second reason is to provide the latest information on advances in such areas as clinical pathology, epidemiology, positron emission tomography, ultrasonography, and environmental causes of esophageal cancer. It has been a true pleasure reading and editing this outstanding collection of articles written by leading authorities in the field.

As I was preparing for this issue, I found some interesting trivia about people who had developed esophageal cancer. Two actors had made comments about what they “jokingly” thought might have been the cause of their esophageal cancer. The first was Humphrey Bogart (1899–1957), who was diagnosed with esophageal cancer in 1956. He was a heavy drinker and cigarette smoker. He had surgery that involved an esophagectomy, removal of two lymph nodes, and a rib, and he also had a course of postsurgical chemotherapy. He had radiation therapy due to recurrence of the cancer 6 months after surgery. He died at home after falling into a coma. His last words were: “I never should have switched from scotch to martinis.” The second was Jack Soo (1917–1979), a Japanese-American actor who was cast in the 1970s television comedy Barney Miller as the laid-back, but very wry, Detective Nick Yemana, who was also responsible for making the awful coffee that everyone in the office had the misfortune to drink every day. Just before he was taken into the operating room before his death, his last words to his Barney Miller co-star Hal Linden were: “It must have been the coffee.” Some other well-known people who had esophageal cancer include Japanese children's author Kenjiro Haitani (1934–2006); University of Miami basketball legend Dick Hickox (1938–2006); actor Makoto “Mako” Iwamatsu (1933–2006); Jaap Penraat (1918–2006), who helped 406 Jews escape the Holocaust; Texas Governor Ann Richards (1933–2006); and Larry Stewart (1948–2007), who anonymously gave away $100 bills to the needy each December as “Secret Santa.”

The individual articles of this issue are grouped into three general areas. The first four articles concern complex interactions of pathogens, host, and therapeutic modalities relevant to surgical infections. Motaz and Cheadle provide an overview of the microorganisms responsible for most surgical infections, Lowry describes the host response to infection, Patel and Malangoni summarize antimicrobial chemotherapy, and Byrnes and Beilman discuss other therapeutic modalities for the treatment of patients who have surgical infections.

The next series of articles focuses on specific infections of interest to surgical practitioners. Kirby and Mazuski outline measures to prevent surgical site infections, and Herscu and Wilson specifically discuss infections occurring after implantation of prosthetic materials. May elaborates on the diagnosis and management of skin and soft tissue infections. Mazuski and Solomkin describe both community-acquired and nosocomial intra-abdominal infections. There follows a series of articles focusing on other infectious complications of surgical therapy: Kieninger and Lipsett, Goede and Coopersmith, and Ksycki and Namias provide detailed information regarding postoperative pneumonia, catheter-related bloodstream infections, and urinary tract infections, respectively. The final article in this section, by Efron and Mazuski, describes Clostridium difficile colitis, a modern pestilence directly related to use and misuse of antibiotics.

Numerous interventions can be used to prevent and treat infections associated with surgical therapy. The ultimate section of this issue attempts to bring together some of those themes. Evans and Sawyer summarize measures to avoid development of resistant bacteria and Fry delineates systems approaches for prevention of surgical infections. Finally, Haas and Nathens describe potential future approaches for the management of surgical infections.

In the end, we, as surgeons, share responsibility for creating many of the modern-day plagues of nosocomial infections. Nevertheless, we also possess tools that can help thwart or ameliorate these infections. What is required is effective use of existing evidence-based practices for the prevention and management of surgical infections. Future investigations will lead to new approaches to control these infections, but these scientific advances will only be of value if they can be integrated into surgical practice. Ultimately, we are indeed our own worst enemies if we choose to ignore the importance of appropriately preventing and treating these infections, which can counteract even our best surgical skills. By conscientiously applying the principles outlined in this issue for managing surgical infections, we can protect our patients from the adverse consequences of these infections, and thereby improve the overall quality of surgical care

The surgical management of venous disease is filled with dichotomies. Procedures span the spectrum from the most mundane saphenous vein stripping (one of the first procedures I did as an intern) to sophisticated central venous reconstructions. Venous surgery tends not to be glamorous, playing second fiddle to arterial reconstruction but frequently is technically more demanding and very dependant on meticulous technique. Diagnosis may be straightforward as in identifying lower extremity varicosities or extremely challenging as in confirming a true thoracic outlet syndrome. Though responsible for a great deal of morbidity in the population as a whole, it receives less attention than it deserves from caregivers. The patient who presents with DVT now frequently is sent home immediately on injectable anticoagulants “per protocol,” deceptively minimizing the long-term sequelae of chronic venous stasis disease. Venous disease is the Rodney Dangerfield of surgery, getting little respect. This is a mistake as Dr. Illig and his colleagues capably illustrate in this issue of Operative Techniques in General Surgery. They show the numerous surgical options available to treat various venous disorders and the evolving nature of those treatments. This broad reaching issue spans newer treatments for the lowly varicosity all the way to the complexities of vena cava and subclavian vein reconstruction. Just as tackling venous disease takes a special kind of surgeon with a special commitment to succeed, so did Dr. Illig take on the challenging task of organizing this issue, something that a less motivated individual might shy away from. He has recruited a truly elite group of surgeons to create this unique issue on this often-minimized surgical topic. I thank him and his contributors sincerely for a valuable compendium that brings to the forefront the surgical options available to the patient with venous disease.

The current issue highlights the experience and the potential value of regional therapy and addresses many of the challenges to cancer therapy. The field continues to evolve and there is definite room for continued innovation not only for tumor access for the anticancer drugs but also for techniques to modify the drug delivery to improve access to the tumor cells themselves, which are often protected by various mechanisms, such as peri-tumor edema, in addition to the unique drug-resistant mechanisms in the tumor cell. The role for coupling biologic agents with chemotherapy to facilitate cancer therapy is under active exploration as is therapy with other agents that are gene derived or genetically engineered to facilitate therapy. These efforts will provide grounding for future development. The current articles highlight many future possibilities and past successes and limitations and we look forward to the continued future development of regional therapy.

In 1981, the Surgical Clinics of North America published an issue entitled “Biliary Tract Disease” . This predated any of our attempts at laparoscopic biliary surgery and was state-of-the-art. In 1994, the Clinics published “Biliary Tract Injuries Revisited,” straddling the era of “all open” biliary surgery and emerging laparoscopic surgery .This issue of the Surgical Clinics of North America will try to add on to the traditions of biliary surgery set forth in previous issues. We have learned much about the mechanisms and prevention of laparoscopic bile duct injuries in the last 20 years, and a review of pertinent anatomy and operative techniques will be presented. Diagnosis, palliation, and surgical management of injuries and pathology of the biliary tract have advanced logarithmically. Radiologic techniques are emerging that provide three-dimensional reconstructions of complex biliary anatomy. Endoscopic imaging and treatment options are evolving as rapidly as laparoscopic instrumentation. All of these new technologies are leading to safer, more effective management of proximal and distal biliary malignancy, biliary reconstruction, and preoperative planning for all biliary surgery. We hope that this issue will prove a useful tool both for surgeons just beginning their practice and more senior surgeons whose careers (sometimes anxiously) have bridged the eras of open and laparoscopic surgery.
